List of Documents Required for Health Insurance Purchase & Claims

Health insurance is a service that takes away the financial burden that you might have to bear in case of a medical emergency at any stage of your life. Medical insurance is not just a tax saving tool, but it can be the best investment that you will make in your life. It is recommended that you purchase a health insurance policy when you are young, typically when you start earning. Ideally, you can get the individual health insurance  plan when you turn 18 years of age. However, if you have still not made this major investment of your life, do not fret. You still have a chance to secure your savings and rely on the health insurance policy to take care of finances, when you avail any health care services. Know everything about medical insurance plans and choose a plan proactively, keeping in mind all your requirements. There are pre-defined sets of documents that you need to supplement while buying insurance, as well as while filing a health insurance claim against your plan. Following is the list of documents needed to apply for health insurance:
  • Proof of Age - You will have to provide the age proof of all the persons who will be getting covered with the health insurance policy opted by you. Following are the acceptable documents you can provide for this:
    • Birth certificate
    • 10th or 12th mark sheet
    • Passport
    • Aadhaar card
    • Voting ID
    • Driving license
    • PAN card etc.
  • Identity Proof - You will be required to produce any of the following identity proof:
    • Aadhaar card
    • Passport
    • Voting ID
    • Driving license
    • PAN card
  • Address Proof - You will be required to submit your permanent proof of address. You can submit any of the following documents for the same:
    • Electricity bill
    • Telephone bill
    • Ration card
    • Passport
    • Aadhaar card
    • Driving license
    • Voting ID
  • Passport size photos
  • Sometimes health insurance policies might require you to undergo a few medical tests. You will be required to submit the medical reports detailing the results of these tests as well.
Besides these documents, insurance companies might require some specific set of documents or other relevant information based on their business rules, and also on the scrutiny of your proposal. The documents needed for a claim are different than the ones needed for registering a claim. If you go for a cashless health insurance , you need not worry about submitting any documents to the insurance company. The network hospital, where you received the treatment will send all the required documents as well as details of your treatment to your health insurance company. However, in case you opt for claim settlement by reimbursement, you will need to submit the necessary documents to your insurance company, by collecting them from the hospital where you received the treatment. The insurance company will verify all the documents submitted by you and the claim amount will be directly deposited in your bank account. Following is the list of documents required for health insurance claim settlement by reimbursement:
  • Insurance company's claim form, duly filled and signed by you
  • Discharge card
  • Written consultation from the doctor with receipts
  • Your hospital bills, which have to be stamped and signed by the hospital authorities
  • X-ray films and other test results such as blood test, urine test etc.
  • Medicine bills
  • Other relevant documents relating to the cause of treatment
It is best advised that you maintain a copy of all the documents that you have submitted while applying for a health insurance policy and while registering a claim against the same.
