Reimbursement cover for all sections

Reimbursement Claim Process

In case of accidental injury or death, you can claim your settlement using the reimbursement process. Here are the documents required depending on the cover you have claimed for:

Documents required in case of death:

  • A duly completed claim form signed by the nominee/legal heir of the insured person.
  • A copy of address proof (Aadhaar/PAN card).
  • Attested copy of the death certificate.
  • Attested copy of the FIR/Panchanama/Inquest Panchanama.
  • Attested copy of the post-mortem report.
  • Attested copy of the viscera report. (Only if it is preserved and sent for further analysis that is mentioned on the post-mortem report).
  • NEFT details and cancelled cheque of the nominee/legal heir of the insured person.
  • A copy of your Aadhaar card, or any other government photo ID and PAN Card. This is not mandatory if your ID card is linked with the policy while issuance or in a previous claim.

Documents required in case of disappearance:

  • A duly completed claim form signed by the nominee/legal heir of the insured person.
  • A copy of address proof (ration card or electricity bill).
  • Attested copy confirming disappearance from appropriate authority following a forced landing, stranding, sinking or wrecking of a conveyance.
  • Documentary proof of fare paying passenger of a conveyance as an occupant.
  • Attested copy of the statement of witness, if any lodged with police authorities.
  • Attested copy of FIR/Panchanama/Inquest Panchanama.
  • NEFT details and cancelled cheque of the nominee/legal heir of the insured person.
  • Original policy copy along with the original assignment endorsement (if any).
  • A copy of your Aadhaar card, or any other government photo ID and PAN Card. This is not mandatory if your ID card is linked with the policy while issuance or in a previous claim.

Documents required in case of ambulance cover:

  • A duly completed claim form signed by the claimant.
  • Original bills and receipts paid for the transportation from a registered ambulance service provider.
  • Other documents as may be required by Bajaj Allianz General Insurance to process the claim.
  • A copy of your Aadhaar card, or any other government photo ID and PAN Card. This is not mandatory if your ID card is linked with the policy while issuance or in a previous claim.

Documents required for permanent total disability and permanent partial disability:

  • A duly completed claim form signed by the insured person.
  • Attested copy of the disability certificate from a civil surgeon of a government hospital stating percentage of disability.
  • Attested copy of the FIR. (if required)
  • All X-ray/investigation reports and films supporting the disability.
  • NEFT details and cancelled cheque of the insured person.
  • Photograph of the patient before and after the accident to support the disability.
  • A copy of your Aadhaar card, or any other government photo ID and PAN Card. This is not mandatory if your ID card is linked with the policy while issuance or in a previous claim.

List of claim documents specific to children education benefit:

  • A bonafide certificate from the school/college or certificate from the educational institution/birth certificate.
  • A copy of your Aadhaar card, or any other government photo ID and PAN Card. This is not mandatory if your ID card is linked with the policy while issuance or in a previous claim.

List of claim documents specific to adventure sports benefit:

  • A certificate of participation from the sports event organiser/service provider.
  • Pre-participation fitness certificate.
  • A certificate from the treating doctor mentioning the nature of the injury.
  • All investigation reports.
  • Discharge summary (if hospitalised).
  • Documents as listed for claim under death/permanent total disability.
  • A copy of your Aadhaar card, or any other government photo ID and PAN Card. This is not mandatory if your ID card is linked with the policy while issuance or in a previous claim.

List of claim documents specific to fracture care cover:

  • X-ray confirming the fracture and site of fracture.
  • A certificate from the treating surgeon mentioning the extent of the injury, cause of injury, site of injury and date of injury.
  • Treatment details.
  • Discharge summary (if hospitalised).
  • Letter from HR with leave record in case of a salaried individual.
  • A copy of your Aadhaar card, or any other government photo ID and PAN Card. This is not mandatory if your ID card is linked with the policy while issuance or in a previous claim.

List of claim documents specific to EMI payment cover:

  • Current outstanding loan certificate from financer along with the documents submitted for the same.
  • Attested copy of the repayment schedule with balance outstanding loan amount statement as on date of loss.
  • All X-ray/investigation reports and films supporting the disability.
  • The claim form with NEFT details and cancelled cheque duly signed by the insured person.
  • A copy of your Aadhaar card, or any other government photo ID and PAN Card. This is not mandatory if your ID card is linked with the policy while issuance or in a previous claim.

List of claim documents specific to hospital cash benefit cover:

  • A duly completed claim form signed by the claimant.
  • A copy of the hospital discharge card.
  • A copy of the hospital bill, money receipt, duly signed with a revenue stamp card.
  • All laboratory and diagnostic test reports for example: X-ray, ECG, USG, and MRI scan, haemogram, etc.
  • Other documents as may be required by Bajaj Allianz General Insurance to process the claim.
  • A copy of your Aadhaar card, or any other government photo ID and PAN Card. This is not mandatory if your ID card is linked with the policy while issuance or in a previous claim.

List of claim documents specific to loan protector cover:

  • Loan disbursement letter along with the payment record till the date of accident.
  • Attested copy of the repayment schedule with balance outstanding loan amount statement as on date of loss.
  • Attested copy of the death certificate.
  • All X-ray/investigation reports and films supporting the disability.
  • A claim form with NEFT details and cancelled cheque duly signed by the insured or his/her nominee in case of insured s death.
  • A copy of your Aadhaar card, or any other government photo ID and PAN Card. This is not mandatory if your ID card is linked with the policy while issuance or in a previous claim.

List of claim documents specific to loss of income due to disability from accident cover:

  • Duly completed personal accident claim form signed by the insured.
  • Attested copy of FIR (if required).
  • All X-ray/investigation reports and films supporting the disability.
  • The claim form with NEFT details and cancelled cheque duly signed by the insured.
  • For employed individuals; leave certificate from the employer mentioning the exact leave period, duly signed and sealed by the employer.
  • Final medical fitness certificate from the treating doctor stating the type of disability, period of disability and declaring the patient to be fit to resume his duties.
  • A copy of your Aadhaar card, or any other government photo ID and PAN Card. This is not mandatory if your ID card is linked with the policy while issuance or in a previous claim.

List of claim document specific to road ambulance cover

  • Duly completed claim form signed by the claimant.
  • Original bills and receipts paid for the transportation from the registered ambulance service provider.
  • A certificate from the treating doctor to transfer the injured person to a higher medical centre for further treatment.
  • All laboratory and diagnostic test reports for example, X-ray, ECG, USG, and MRI scan, haemogram, etc.
  • Other documents as may be required by Bajaj Allianz General Insurance to process the claim.
  • A copy of your Aadhaar card, or any other government photo ID and PAN Card. This is not mandatory if your ID card is linked with the policy while issuance or in a previous claim.

List of claim document specific to travel expenses benefit cover:

  • Original travel tickets/bills and receipts mentioning the actual expenses of the travel with the date of booking and date of travel.
  • First consultation letter from the doctor.
  • Duly completed claim form signed by the claimant.
  • A copy of your Aadhaar card, or any other government photo ID and PAN Card. This is not mandatory if your ID card is linked with the policy while issuance or in a previous claim.

List of claim documents specific for accidental hospitalisation expenses:

  • First consultation letter from the doctor.
  • Duly completed claim form signed by the claimant.
  • Original hospital discharge card.
  • Original hospital bill giving detailed break-up of all expense heads mentioned in the bill. Clear break-ups have to be mentioned for OT charges, doctor s consultation and visit charges, OT consumables, transfusions, room rent, etc.
  • Original money receipt duly signed with a revenue stamp.
  • All laboratory and diagnostic test reports for example, X-ray, ECG, USG, and MRI scan, haemogram, etc.
  • Other documents as may be required by Bajaj Allianz General Insurance to process the claim.

Cashless Claim Process

Cashless treatment is only available at network hospitals. In order to avail the cashless treatment, follow the below mentioned procedure:

a. Prior to taking treatment and/or incurring medical expenses for any accidental injury at a network hospital, you must call us and request pre-authorisation by way of the written form provided by us. Waiver of this condition shall be considered in case of emergency hospitalisation arising out of an accidental bodily injury.

b. After considering your request and after obtaining the complete information and documentation, we will send an authorisation letter to you or the network hospital. The authorisation letter, your policy ID card and any other information or documentation specified by us must be produced to the network hospital identified in the pre-authorisation letter at the time of your admission to the hospital.

c. If the above procedure is followed, you will not be required to directly pay for the medical expenses at a network hospital in case of an accidental injury. The hospital will be indemnified by us under accidental hospitalisation expenses and the original bills and proof of treatment will be left with the hospital. However, pre-authorisation does not guarantee that all costs and expenses will be covered. We reserve the right to review each claim for medical expenses and accordingly coverage will be determined according to the terms and conditions of this policy. You will, in any event, be required to settle all other expenses directly.

What is Global Personal Guard policy?

Global Personal Guard policy is an insurance plan that provides extensive global coverage in case of death, disability or injury caused due to accidents.

Why should I have a personal accident cover?

Personal accident policies provide you financial support if you are disabled or injured after an accident. Unexpected medical expenses can be a major setback financially. A comprehensive personal accident insurance allows you and your family to be financially secure after an unforeseen event.

How is Global Personal Guard policy different from other personal accident policies?

A standalone personal accident policy covers you and your family from accidental death, permanent disability and partial disability. However, you remain unprotected from hospitalisation expenses, ambulance charges, loan liability, physiotherapy and other such expenses. Global Personal Guard policy covers all these expenses along with additional benefits like loss of income cover, fracture cover, adventure sports benefit, daily cash benefit, and travel expenses benefit all under one policy that assures financial support and complete peace of mind in case of an unfortunate accident.

Does the Global Personal Guard policy cover natural death or death due to some illness/disease?

Global Personal Guard policy only covers death occurring due to accidents or accidental injuries.

What are the benefits available under base cover?

The base cover consists of death, total permanent disability and partial permanent disability.
Death: Death benefit in the event of an accidental death. In addition to death cover, additional benefits such as:

  1. Transportation of mortal remains - paid as 1% of the sum insured selected under death.
  2. Funeral expenses - paid as 1% of the sum insured selected under death.

Both the benefits are over and above the sum insured opted.

Also, we provide a disappearance cover. For example, if the person is travelling in a conveyance that suffers a forced landing, stranding, sinking or wrecking and disappears due to an accident, then after 12 months of disappearance, it will be assumed that the person has died as the result of the accident and the benefit will be payable to the nominee.

Total permanent disability:

Total permanent disability benefit in the event of an accidental bodily injury resulting in:

  1. Loss of sight in both eyes.
  2. Physical separation of or the loss of ability to use both hands and both feet.
  3. Physical separation of or the loss of ability to use one hand and one foot.
  4. Loss of sight of one eye and the physical separation of or the loss of ability to use either one hand or one foot.

In addition to the above, a lifestyle modification benefit of 2% of the sum insured will be paid to the insured person. This benefit is over and above the sum insured opted.

Permanent partial disability:

If you sustain accidental bodily injury during the policy period which directly and independently of all other causes results in permanent partial disability within 12 months from the date of accident, then a percentage of the sum insured is payable to you as shown below:

What does accidental hospitalisation cover?

Accidental hospitalisation covers in-patient treatment and medical expenses due to hospitalisation for a minimum of 24 hours because of an accidental injury.
