How to File Maturity Claim?

Three Simple Steps to File Maturity Claim

Step 1: Claim Intimation

  • Bajaj Allianz would send intimation letter to the customer 6 months before the maturity date along with maturity claim discharge form & annuity form (if pension policy).

Step 2: Submission of Documents

Visit the nearest branch /courier the below listed documents:

  • Maturity Discharge Form
  • Original policy document
  • Standard KYC documents (eg. Identity Proof and Address Proof)
  • Bank supporting documents
  • NEFT Form (sealed by bank)
  • Cancelled cheque containing IFSC code, Account No. Customer Name (should be printed on cheque) (OR) Latest six months bank account statement (sealed by bank)
  • Annuity Form (in case of Pension Policy)

Step 3: Settlement of Claim

  • On compliance of all the above documents, the maturity claim amount shall be paid.
  • For Pension Policy , the amount directed by customer towards purchase of Annuity would be sanctioned in favor of the Annuity provider (the insurer from whom the Immediate Annuity would be bought) and remaining amount would be credited to the policyholder's account

How to File Death Claim?


Things to remember
  • Read your policy document thoroughly.
  • Verify all the details featuring on your policy document and inform us immediately in case of any discrepancies.
  • Do not keep life insurance policies in your safe deposit box. In most states, safe deposit boxes are sealed temporarily upon the death of the owner, which could delay settlement.
  • Do not forget to note down the policy number and keep it safe.
  • Do not forget to inform us incase of any change in address /contact information.

Three Simple Steps to File Death Claim

Step 1: Claim Intimation

  • Visit the nearest office with a copy of death certificate, issued from the Birth and Death Registrar.
  • Provide KYC documents (Proof of Address and Identity) of nominees /claimant upon which a bar coded claim form will be generated by our office which needs to be submitted along with the other documents (as mentioned in Step 2).

Step 2: Submission of Claim Form

Provide the following documents to process claim:

  • Filled in bar coded claim forms issued by our office.
  • Original policy document
  • Death certificate.
  • Police FIR and post mortem exam report (for accidental death).
  • Certificate and records from the treating doctor/hospital
  • Claims ECS Mandate Form and cancelled cheque leaf.
  • Additional documents, if required.

Step 3: Settlement of Claim

  • On receiving of all the above necessary documents the claim settlement process will start within seven working days

How to File Rider* Claim

(that arises due to health or disability)

Step 1: Claim Intimation

Visit the nearest branch with

  • The medical examination (diagnosis) reports and /or discharge summary in support of the illness of the insured person. (in case of Critical Illness and Mahila Gain).
  • Available medical documents in support of disability of assured, along with a full size photograph (in case of Accident Permanent Total /Partial Disability) (APTPDB).
  • The discharge summary and bifurcated hopsitalisation bills showing room rent /bed-charges separately (in case of Hospital Cash)

Provide the original policy document and the standard KYC documents (proof of address & identity) of claimant upon which a bar coded claim form will be generated by the branch which needs to be submitted along with the other documents (as mentioned in Step 2)

Step 2: Submission of Claim Form

Provide the following documents to process claim:

  • Filled in bar coded claim forms issued by the insurance company.
  • Medical case sheets from the treating doctor/hospital (in case of both CI & APDB).
  • Police FIR or any other document in support of the disability to be outcome of an accident (in case of APDB).
  • ECS mandate form /bank passbook copy /cancelled cheque leaf of the claimant’s bank account.
  • NEFT Form (sealed by bank)
  • Cancelled cheque containing IFSC code, Account No. Customer Name (should be printed on cheque) (OR) Latest six months bank account statement (sealed by bank)
  • Additional documents, if required by the insurance company.

Step 3: Settlement of Claim

  • On receiving of all the above mandatory documents the claim settlement process will start within seven working days.

* Note

The riders in which claim arises due to health or disability are as follows:
Critical Illness, Mahila Gain, Accident Permanent Total /Partial Disability, Hospital Cash Benefit, Family Income Benefit, Comprehensive Accident Protection, Waiver of Premium.

For rider claims that arise due to death, Death Claim Process is to be followed.
These riders are as follows:
Accidental Death benefit, Term Rider / Supplementary Death Benefit, Family Income Benefit, Waiver of Premium, Comprehensive Accident Protection

Car and Two Wheeler Insurance Claim

To register an insurance claim with us, you need to follow a simple and easy process.

Once you register your claim with us on the Toll Free Number - 1800-209-5858, you will receive SMS updates to guide you through each stage of the process. Follow the steps provided below to understand our insurance claims process in detail.

Step 1: Send your Vehicle for Repair

In case of an accident, take your vehicle to the garage (if movable) or have it towed to avoid further damage.

For Theft Claims: In case of a theft claim, file a police complaint immediately in writing and inform us on our Toll Free Number - 1800-209-5858. If your vehicle isn't found within 90 days, ask the police to issue you a Non-Traceable Report. This is an undertaking given by the police that they are yet to find your vehicle; you need to submit this report to us. We start the claim process after this.

Step 2: Register your claim

Register your claim by calling us on our Toll Free Number: 1800-209-5858
You will need to provide the following information to the customer support executive:

  • Contact number
  • Engine and Chassis Number
  • Accident date and time
  • Description and location of the accident
  • Vehicle inspection address
  • Km. reading

Once the claim is registered, the customer support executive will provide you with a Claim Reference Number. You will be updated through SMS on every stage of the claim OR you can call on our Toll Free Number - 1800-209-5858 and quote the Claim Reference Number to know the status of your claim.

Step 3: Survey and Claim Settlement

Documents Required: Submit the copy of documents to the garage / dealer and get it verified with the originals.

Spot Settlement: If the loss is not much, we suggest that you avail spot settlement of claims through Bajaj Allianz Mobile Settlement Service.

Claim Settlement:
  • After completion of repair at a preferred garage we will make payment of the loss directly to the garage.
  • You will only have to pay the excess as mentioned in the policy and the depreciation value, salvage etc. as informed by the surveyor.
Health Insurance Claim process

Life Health Insurance claim
Household goods in transit
  • In case any damage is suspected in transit, open delivery should be insisted upon the carrier and their certificate should be obtained.
  • In case of loss/damage in transit, a monetary claim should be lodged with the carrier within the time limit to protect recovery rights, without which, the claim may not be admitted.
Health Guard Insurance Claim (for non-cashless claims)
  • Notice of claim should be lodged within 24 hours.
  • The insured should submit 'discharge summary' of the hospital/nursing home along with original hospital/medical bills,reports of the labs and investigation reports.In other words every item in the claim bill should be supported.
  • Leave certificate from the employer, wherever needed.
  • Fitness certificate from the Doctor.
Travel Companion Insurance Claim

Our travel claim process is simple and easy!

Please note that it is important to notify us before seeking any medical consultation while you are travelling abroad (unless it is an accident or emergency). We have a coordinating doctor on duty to speak to you and provide medical advice. We can also provide medical referral for any specialist doctor or hospital.

Hospitalisation Claims

If you are admitted to a hospital after OPD consultation or otherwise please notify us immediately.

The Insurance medical assistance department (doctor) will discuss your medical condition with the Treating doctor and if it is confirmed that the admission to a hospital is NOT due to any pre-existing conditions or any conditions listed in the exclusion list, then the Insurance Company shall guarantee payments to the hospital and settle the payments with the hospital.

Documents Required: In order to expedite processing of the claim you must send the following documents immediately by

  • Completed Claim Form signed by the Treating Doctor and you.
  • Name, address and contact details of the Local / Family Medical Doctor in India.
  • Name, address and contact details of the Local / Family Medical Doctor in India.
  • Signed copy of the RELEASE OF MEDICAL INFORMATION FORM (ROMIF) to authorize your Treating Doctor and Local Medical Doctor in India to release your medical information to the Insurance Medical Assistance Department
  • Treating Doctor's medical report.
  • Date of Travel from India.
  • Details of domestic health Insurance policy, if any (Policy No, name and address of insurance company).
  • Copy of passport with exit stamp from India and entry into overseas country.

Your prompt submission of the above documents / information (ROMIF, LMO details, Medical Reports, estimated medical expenses, Claim Form) will enable the Medical Assistance Department to make a medical assessment and recommendation of coverage thereby facilitating the claims process.

Outpatient Consultation Claims

For outpatient consultation, you will have to self-pay and file the claim upon return to India. However, if the amount is above USD 500 and you are facing difficulty in making payments due to shortage of cash on hand then we can arrange to make payments on behalf of you, provided the medical condition is confirmed to be non-pre-existing.

Documents Required: The important documents required to file a claim is as follows :

  • Claim Form (To be signed by the Treating Doctor and Insured), with the Claims Reference Number.
  • Doctor's medical report.
  • Original admission/discharge card.
  • Original bills, receipts and prescriptions.
  • Original X-ray, pathological and investigative reports.

Important: If any hospital does not submit a bill to you for the treatment or service rendered, please inform us before leaving the hospital. If the hospital insists that they will claim directly from the Insurance Company, you should inform them that Bajaj Allianz will not entertain any such requests from the hospital. Only claims where the insured files directly (with all relevant documents) on return to India will be considered.

Please send all documents to the address of Notification of Claims, as mentioned in the policy schedule.

Documents Required for Different Types of Claims

Medical Accident and Sickness Expenses (outside India)

Documents Required:
  • Claim Form (To be signed by the Treating Doctor and you) and with the Claims Reference No.
  • ROMIF
  • Doctor's medical report.
  • Original admission/discharge card.
  • Original bills, receipts and prescriptions.
  • Original X-ray, pathological and investigative reports.
  • Copy of passport, visa with entry and exit stamp.

Procedure:

  • Please contact our Assistance Department at the number given above, obtain a Claim form from them and fill it in. It is necessary to obtain the attending physician's signature on the Form.
  • Please collect all bills/Receipts and Invoices.
  • Submit all documents to the address mentioned on the policy schedule.

Loss of Baggage

Documents Required:
  • Signed Claim Form
  • Copies of boarding Pass/Ticket/Baggage Tags
  • Copies of correspondence with the Airline authorities/others certifying the delay
  • PIR Report (to be obtained from the airline authorities)
  • Details of compensation received from Airlines/other authorities
  • Bills and receipts of purchase of emergency toiletries and medicines
Procedure:
  • Obtain confirmation of the delay from the airline.
  • Claim Form can be obtained from our Assistance Department
  • Fill in the Claim Form and submit all documents to the address mentioned on the policy schedule.

Delay of Baggage

Documents Required:
  • Signed Claim Form
  • Copies of boarding Pass/Ticket/Baggage Tags
  • Copies of correspondence with the Airline authorities/others certifying the delay
  • PIR Report (to be obtained from the airline authorities)
  • Details of compensation received from Airlines/other authorities
  • Bills and receipts of purchase of emergency toiletries and medicines
Procedure:
  • Obtain confirmation of the delay from the airline.
  • Claim Form can be obtained from our Assistance Department
  • Fill in the Claim Form and submit all documents to the address mentioned on the policy schedule.

Loss of Passport

Documents Required:
  • Copy of new passport
  • Copy of previous passport (if available)
  • Copy of return tickets
  • Proof of complaint to the local police
  • Details of expenses incurred in obtaining fresh passport
Procedure:
  • File a complaint with the local police
  • Contact the Indian Embassy, wherever necessary
  • Submit all documents to the address mentioned on the policy schedule

Tuition Fee

Documents Required:
  • Duly filled in claim form
  • Copy of the receipt of payment of advance tuition fee
  • Letter informing the school regarding the inability of the student to continue the semester duly acknowledged by the school.
  • Discharge card of hospitalization along with detailed medical report and all investigation reports
Procedure:
  • Obtain confirmation of inability to attend school from the school authorities
  • Claim form can be obtained from our Assistance Department
  • Fill in the Claim form and submit all documents to the address mentioned on the policy schedule

Accident to Sponsor

Documents Required:
  • Duly filled personal accident claim form.
  • Copy of the death certificate of the deceased.
  • Death Summary from the hospital where last treated.
  • Letter from the school authorities intimating about the balance tuition fee if any for the course
  • Any other documents if required will be informed by the claims department of Bajaj Allianz
Procedure:
  • Inform our Assistance Department regarding the incident
  • Submit all documents to the address mentioned on the policy schedule

Family Visit

Documents Required:
  • Duly filled Medical Sickness or Personal Accident claim form.
  • Certificate from the doctor treating the you mentioning the requirement of an attendant and reasons for the same.
  • Travel tickets in original along with the copy of the visa and passport.
Procedure:
  • Inform our Assistance Department regarding the condition of the you.
  • Obtain prior authorization that the travel of the family member will be covered.
  • Submit all documents to the address mentioned on the policy schedule.

Personal Liability

Documents Required:
  • Full statement of the facts in writing.
  • Witness statements
  • Any other documents relevant to the incident, including Summons, Legal Notice, etc.
Procedure:
  • Inform our Claims Dept. immediately (at the address given on the policy schedule below) giving full details of the incident.
  • Do not commit any benefit/compensation or enter into an agreement.
